The Vermont minor child power of attorney form is a document that would allow a parent or legal guardian to grant powers to another person to serve as their Attorney In Fact/Agent. The Agent would provide proper temporary care for the child(ren) in the absence of the parent(s) or guardian. Complete the document and have it acknowledged by a notary public. At the discretion of the parent or guardian, the form may be revoked.
How to Write
Step 1 – The Parties – Enter the following:
- Name of the Parent or Guardian
- Complete address
- Enter the name of the appointed temporary guardian
- Complete address
Step 2 – Relationship to Child(ren) –
- Select the applicable box
- Enter the name(s) of the child(ren)
- Read the remaining statement
Step 3 – Date of Document Termination –
- Enter the termination date of the document within one year in dd/mm/yy format
- Submit parent or guardian’s signature
Step 4 – Notarization –
- The remainder of the document shall be completed by a Notary Public
