Michigan Revocation Power of Attorney Form

The Michigan revocation power of attorney form is a document that will allow a Principal to revoke a document that was previously completed allowing an Agent powers of any type. Completion and delivery of this document to the Agent, will allow revocation of any previous powers document.

This document must be witnessed by a Notary Public

How to Write

Step 1 – The Parties –

  • Enter the name of the Principal
  • Enter the name of the Agent to whom the revocation document will be delivered
  • Enter the date that the powers document was created in dd/mm/yyyy format

Step 2 – Signature –

  • Enter the Principal’s signature

Step 3 – Notary Acknowledgement

Once the Notary has witnessed the signature required, they shall complete the remainder of the document in acknowledgement
