Montana Vehicle Power of Attorney Form | |
The Montana vehicle power of attorney form is a legal document in which a Principal or company will have the ability to grant powers to an Agent to sell, buy and/or do transactions on behalf of the Principal and/or in their absence. This document, once executed will remain in effect until the act is complete.
The document will require notarization. This document may also be revoked by the Principal by placing notice in writing and delivered to the stated Agent.
How to Write
Step 1 – Vehicle Owner (Principal’s) Information – Complete the following:
- Print the Principal’s legal name
- Address, City, State and Zip Code
- AND
- Enter the name of the designated Agent
- Address
- City
- State
- Zip Code
- Read the remaining statement and enter any other power(s) that will be allowed by the Principal
Step 2 – Vehicle Information – Enter the following information:
- Title Number
- Year
- Make
- Model
- Vehicle Identification Number (VIN)
- Color
- License
- Plate Number
Step 3 – Odometer Reading – Read the statement provided and complete the following:
- Check the circle indicating whether the VIN is 5 or 6 numbers
- Enter the odometer reading (no 10ths of a mile)
- Enter the date in which the odometer was read
- If the Principal is knowledgeable that the odometer may not be accurate, check the box that best indicates any possible discrepancies or excess of required mileage
Step 4 – Principal’s Signature – Must be signed before a Notary:
- In the presence of a notary public, the Principal must submit their signature
- Enter the date of the signature in mm/dd/yyyy format
Step 5 – Notarization –
When the Notary Public witnesses the signature, they shall then complete the Notary section in acknowledgement of the validity of the document and signature
Step 6 – Purchaser – The purchaser (if applicable) must certify that the odometer reading submitted by the owner is correct:
- Purchaser’s signature (only one signature is required)
- Purchaser’s printed name
- Date of signature in mm/dd/yyyy format
- Should the applicant is a Firm or Corporation, Print or Type the full name in the line provided on the form
