North Dakota Revocation Power of Attorney Form | |
The North Dakota revocation power of attorney form is a document that is designed to permit a Principal to provide written notice stating their decision to revocation of a previously written powers document. Use of this document shall immediately, as of the date stated on the revocation form, release the Agent from all of the responsibilities of their powers. The document must be delivered to or served to the Agent and all others who hold copy of the stated document.
This signatures must be entered before a Notary Public.
How to Write
Step 1 – Check the appropriate boxes then enter the following information:
- The name of the Principal
- The Agent’s full name
- Submit the date that the document was executed
Step 2 – Signature –
- The Principal must read the statement provided
- Submit the date of execution, of the revocation document in mm/dd/yyyy format
- The Principal must enter their signature
Step 3 – Notarization –
When the Principal completes the document and the Notary has witnessed the Principal’s signature, the Notary will then complete the remainder of the document in acknowledgement and shall affix their official seal
