New Hampshire Revocation Power of Attorney Form | |
The New Hampshire revocation power of attorney form is a document that is designed to provide written revocation notice from a Principal to an Agent. This document allows the Principal to remain in compliance with providing written notice to the Agent when the Principal feels that the Agent’s services are no longer needed. The Principal, once the document is completed, must have the document delivered or served to the Agent to ensure that they are properly notified.
How to Write
Step 1 – Download the Document – Provide the following information:
- The Principal’s full name
- The Principal’s City of Residence
- County
- AND
- Enter the recorded date of the document to be revoked
- Enter the name of the Agent to whom powers were granted, but will now be revoked
Step 2 – Date of Revocation, Principal’s Signature and Witnesses –
- Enter the date in which the Principal would like the revocation to be recognized. The Principal may enter the same date of the preparation of the revocation document in mm/dd/yyyy format
- Enter the signature of the Principal
- The witnesses must provide their signatures
- Enter the witnesses mailing addresses
Step 3 – Notary Acknowledgement –
- Once the document is completed, the notary will witness all signatures and complete the document in acknowledgement
