Nevada Minor Child Power of Attorney Form

The Nevada minor child power of attorney form is a document that is utilized by a parent or parents to provide temporary guardianship over their child(ren) in their absence. The document will temporarily grant care and custody to a trusted Guardian selected by the Parent(s). This document includes all of the needed and reasonably required care to include medical, dental, surgical, food, clothing etc.

If both parents are married an co-habitating then the document will represent the permission of both parents. If the parents are divorced, then the custodial parent may make these designations on their own. This document will be executed for a six (6) month period only.

This document must be signed before a Notary Public. This document may be revoked by the parent(s) or the courts within the jurisdiction of the Guardian any time, by providing written notice to the Guardian.

How to Write

Step 1 – The Parties – Submit the following:

  • Names of the Parent(s)
  • Enter the name(s) of the Child(ren)
  • Enter the name(s) of the Guardian(s)
  • The Parent(s) and Guardian(s) must read the remaining information

Step 2 – Signatures – Enter the following:

  • The Parent(s) signature(s)
  • Parent(s) Complete Physical Address

Notary for Parent(s) Signatures –

  • The Notary Public will witness the Parent(s) signatures and complete the remainder of the Notary’s information in acknowledgement
  • Enter the Guardian(s) Signature(s)
  • The complete address of the Guardian(s)

Notary for Guardian(s) –

  • The Notary Public will witness the Guardian(s) signatures and complete the remainder of the Notary’s information in acknowledgement
  • Enter the Guardian(s) Signature(s)
  • The complete address of the Guardian(s)

Step 3 – Signature(s) of Minor Child(ren) –

  • Any child(ren) 14 years old or older must provide signatures consenting to the selected Guardian(s) for whom they shall remain in their care.
