Pennsylvania Vehicle Power of Attorney Form

The Pennsylvania vehicle power of attorney form is a document that allows a Principal (owner of a vehicle) to appoint an Agent to complete any transaction with regard to purchase or sales of a motor vehicle.  The document will allow the Agent to sign on behalf of the Principal(s) that are required by the Pennsylvania Department of Transportation. This document must be witnessed and signed by a notary. This form may be revoked any time the Principal finds it necessary.

How to Write

Step 1 – The Parties – Enter:

  • Name(s) of the Principal(s)
  • The name of the Agent(s)

Step 2 – Agent Information –

  • Last four digits of Social Security Number (SSN) if required
  • Driver’s License
  • Complete address
  • Zip Code
  • Transfer of Plate, if any

Step 3 – Vehicle Information – Provide the following:

  • Make
  • Model
  • Year
  • Vehicle Identification Number (VIN)

Step 4 – Signature and Notarization –

  • Notary shall witness signatures
  • Owner’s signature
  • Driver’s license number
  • Co-Owner’s signature
  • Driver’s license number
  • Notary shall complete the form
