Tennessee Vehicle Power of Attorney Form

The Tennessee vehicle power of attorney form is a document that is completed by an individual or business, to assign an Agent the ability to transact change of ownership of one vehicle as shall be stated by the Principal in the document. The Principal will provide the information needed with regard to what powers the Agent will have permission to act upon.

Signatures must be submitted in the presence of a state licensed Notary Public. This document may be revoked at any time, in writing and by delivery or service of the Principal to the Agent.

How to Write

Step 1 – Principal’s Appointment of Agent – Enter the following information:

  • Date the execution of the document in mm/dd/yyyy format
  • Principal’s full name
  • AND
  • Agent’s full name
  • AND
  • Name of the Business
  • Business Street Address
  • City
  • State
  • Zip Code
  • Parties must read the remaining paragraph

Step 2 – Vehicle Information –

  • Make
  • Vehicle Identification Number (VIN)
  • Model
  • Body Type
  • Year

Step 3 – Authorized Transaction Types – The Principal must check the applicable boxes:

  • Duplicate Title
  • Noting of Lien
  • Request for Verification of Ownership on Vehicles Other: Found Abandoned, Immobile or Unattended
  • Vehicle Information Request
  • Transfer of Title
  • Application for Title and Registration
  • Other (specify any other permitted powers)

Step 4 – Granting of Authority – Must be completed by the Principal – Check the appropriate box:

  • Individual
  • Business (Enter the name of the business)
  • Before a Notary, the Principal must provide the Principal’s signature
  • Print the name of the Principal
  • Physical Address
  • City,State,Zip Code
  • Telephone Number
  • Email Address

Step 5 – Notarization –

Once the document is completed and signature has been entered by the principal, the Notary will require identification for record keeping. The Notary shall then be required to complete the remainder of the form in acknowledgement
