New Jersey Revocation Power of Attorney Form | |
The New Jersey revocation power of attorney form is a legal document that allows any Principal to revoke any form of powers document. The Principal may complete the document and deliver or serve it to the current Agent, so as to provide notice that their services are no longer required and that they’re being served with notice of revocation.
This document must be acknowledged before a Notary Public
How to Write
Step 1 – Parties – Enter the following:
- Principal’s Name
- City of residence
- State of residence
- AND
- Enter the date that the powers document was drawn
- Enter the name of the Agent to whom the document was provided
- Provide the name of the Agent
Step 2 – Date and Signature-
- Enter the date in mm./dd/yyyy
Witnesses Signatures –
- Submit the Witnesses Signatures
- Submit the Witnesses Place of Residence
Notarization –
- When all signatures have been submitted, the notary shall complete the document in acknowledgement
