The Texas revocation power of attorney form is a legal document that allows any Principal. Parent or Legal Custodian (of an adult or child) to revoke the powers of a selected Attorney In Fact/Agent and provide notification to the Agent. Completion of this document would keep the Principal in compliance with most any powers document. The form will remove all powers from any Agent to whom the Principal had previously granted powers. Once the document has been completed, it must be signed before two witnesses as well, a licensed Notary Public, at which point the Principal must deliver or have the document served to the Agent.
How to Write
Step 1 – Location – Enter:
- The name of the County where the document shall be executed
Step 2 – The Parties – The Principal must provide:
- Full name
- Complete Address
- Enter the date of execution of the document in dd/mm/yyyy format
- Principal must enter the name of both the primary and successor agents
Step 3 – Signature – To be signed before a Notary Public:
- Principal must date the signature in dd/mm/yyyy format
- The Principal must submit their signature
Notarization and Witnesses –
- The witnesses must read and agree the attestation statement
- Enter the name of the Principal
- Enter the date of the attestation review and agreement in dd/m/yyyy format
- Each witness must submit:
- Their respective signatures
- Street Address
- City, State and Zip Code
Once the Principal and the Witnesses have submitted their signatures before the Notary Public, the Notary will then complete the required information acknowledging the document’s information and signatures.
