The document is only effective if the parent(s) or legal guardian(s) are readily available. Parent’s or Guardian’s signature are required so that the document will be effective. This document may be revoked at any time.
How to Write
Step 1 – The Parties – Submit the following:
- Parent(s) or legal guardian’s signature(s)
- Country
- Agent’s name
- Child(ren’s) name(s)
- Read the remainder of the section
Step 2 – Titled Sections – Review:
- Titled sections numbered 2 through 8
Step 3 – Execution and Date of Signature – Submit:
- Read the declarations
- Date the signature in mm/dd/yy format
- City
- County
- Parent’s printed name
- Parent’s signature
