The West Virginia vehicle power of attorney form is a document that is authorized by a Principal for an Agent to provide any necessary powers needed to the Agent to properly transfer ownership of a motor vehicle or vessel with the DMV. The document must be witness before a notary public. This document may be revoked at the Principal’s discretion.
How to Write
Step 1 – Owner’s Information – Complete the following:
- Buyer, seller or legal owner’s name

Step 2 – Motor Vehicle, Tailor or Vessel Information – Enter:
- Vehicle identification number (VIN) or Hull identification number (HIN)
- Appointed Agent’s name
- Street address

- Telephone number
- Read the remainder of the section
- Date the document in dd/mm/yyyy format
Step 3 – Signatures – Submit the following:
- Owner’s signature
- Date the signature in mm/dd/yyyy format
Step 4 – Notary Certification –
- Notary will witness signatures
- Notary shall then complete the remainder of the document with required information and official seal
