Louisiana Minor Child Power of Attorney Form | |
The Louisiana minor child power of attorney form is a document that is provided by the parent(s) of {a} minor child(ren) to grant certain powers to a selected Guardian(s) to care for their child(ren) in the event of absence or illness. By providing this document, the parent(s) will not lose rights to their child(ren). This document provides permission for the guardian(s) to provide complete care for the child(ren), (with the exception signing for marriage or adoption of the child(ren)).
This document can be revoked at any time by the parent(s) of the child(ren) by provision of a written and delivered notice to the guardian(s). This document must be notarized.
How to Write
Step 1 – Parties – Enter the following:
- The name(s) of the parent(s)
- Physical Address
- AND
- The name of the elected Agent
- AND
- The name(s) of the child(ren) – If more children must be listed, add another sheet with their information and attach it to this document
- Enter the date(s) of birth of the child(ren)
- Enter the year(s) of birth for the child(ren)
Step – 2 – Signature -Must be signed in the presence of a Notary:
- Enter the date of the parent’s signature in dd/mm/yyyy format
- Parent(s) must provide their signature
Once the Notary Public has witnessed the parent(s) signature(s), they Notary shall enter all required information acknowledging the document.
