Louisiana Vehicle Power of Attorney Form

The Louisiana vehicle power of attorney form is a document that is provided by a Principal to grant powers to an appointed Agent to take over all acts of the Principal with regard to the sale, purchase, inspect, registration, back out or rescind to avoid a sale. This document will be implemented immediately, but will expire once the act is completed. Signatures to this document must be provided in service of a Notary Public. This document may be revoked by the Principal at any time.

How to Write

Step 1 – Vehicle Information – Enter the following:

  • Make
  • Model
  • Year
  • Body
  • Vehicle Identification Number (VIN)
  • Principal mus read the information contained in the document before provision of signature(s)

Step 2 – Signatures – Must be signed before a Notary:

  • Owner/Buyer(s) Signature(s)
  • Owner/Buyer(s) Printed Name(s)
  • Witnesses Signatures
  • Witnesses Printed Names

Step 3 – Notary Acknowledgement – The Notary Public must read the information provided and enter:

  • Name of Witness
  • Printed Name
  • Notary Public
  • Date of Signature
  • Date of Signature
