The South Dakota revocation power of attorney form is a legal document that would permit a Principal to revoke any powers document that had been previously executed. Complete the revocation, have the Principal’s signature witnessed. Deliver or have the document served to the Agent. This action complies with powers documents which will provide notice to the Agent that their service is no longer required and that their powers have been terminated.

How to Write
Step 1 – The Parties –
- Submit the name of the Principal
- Provide the name of the Agent
- Enter date that the powers document had been initially completed in dd/mm/yyyy format
Step 2 – Signature –
- Enter the Principal’s signature
Step 3 – Notary Acknowledgement –
Once the Notary has witnessed the completion and signature of the Principal to the document, the notary will complete the remaining portion, in acknowledgement as required by the state.
