West Virginia Revocation Power of Attorney Form

The West Virginia revocation power of attorney form is a document that is used by a Principal who has completed a powers document. For their own reasons, they have decided to revoke the Agent’s rights to any decision making powers, according to the document that was executed. The Principal must complete the document and either deliver the revocation or have it served to the Agent. This document will not be considered effective until the Agent is in possession of the completed document. The signature on this form must be witnessed by a notary.

How to Write

Step 1 – Use of Document –

  • Check the box that would best indicate which powers document shall be revoked, if selecting “Other” enter the title of the powers document

Step 2 – Parties – Submit the following:

  • Principal’s full name
  • Title of the document for revocation
  • Form was executed on (date)
  • Agent’s name
  • Alternate Agent’s name (if any)

Step 3 – Principal’s Signature – Enter:

  • Date of signature in dd/mm/yy format
  • Principal’s printed name
  • Signature
  • Provide copies to Agents and any other interested parties (ie: banks, utilities, etc.)

Step 4 – Notarization –

  • Once the notary has witnessed the signature, they shall provide the remainder of the information that will acknowledge the document
