West Virginia Vehicle Power of Attorney Form

The West Virginia vehicle power of attorney form is a document that is authorized by a Principal for an Agent to provide any necessary powers needed to the Agent to properly transfer ownership of a motor vehicle or vessel with the DMV. The document must be witness before a notary public. This document may be revoked at the Principal’s discretion.

How to Write

  • Date the document in dd/mm/yyyy format

Step 3 – Signatures – Submit the following:

  • Owner’s signature
  • Date the signature in mm/dd/yyyy format

Step 4 – Notary Certification –

  • Notary will witness signatures
  • Notary shall then complete the remainder of the document with required information and official seal
