The Connecticut vehicle power of attorney form is a document to authorize, according to Connecticut law, another person designated by you (the Principal) to make, to complete and to sign on your behalf, an application for a motor vehicle registration and certificate of title, or other written document, form or application that is required to be filed with the Commissioner of Motor Vehicles in connection with any other transactions involving a motor vehicle registration or title. How To Write
Step 1 – Begin by downloading the form and reviewing the information at the top of the page. You would then provide –
- In the first blank line the Principal must print their full name
- The second line is for the printed name of the Attorney In Fact/Agent
Step 2 – Prior to entering any signatures, you must retain the services of a notary public and acquire two witnesses
- Once you’re before the notary enter the date in dd/mm/yyyy format
- The principal must then provide their full name signature
Step 3 – Witnesses – In order to complete this document, there must be two witnesses available to sign this form before the notary –
- Witnesses 1 and 2 must place their signatures on the Witness lines provided
Step 4 – Notarization – The remainder of the form will be completed by the notary and their state seal will be affixed on the document
- Once completed by the notary, the notary will hand the document back to the principal
- Make copies to provide to all who have signed this document
- The original must be sent with the title of the vehicle, no copies – the original will be returned to the principal
