Kentucky Durable Financial Power of Attorney Form | |
The Kentucky durable financial power of attorney form is a document that is considered broad and sweeping inasmuch as it provides powers to an elected Agent that will touch all financial, personal property, real property and more, in the event of illness or while the Principal is in temporary absentia. The Principal must read through the document to be certain they understand the language and what it means. If they are unclear, it’s recommended that the Principal speak with an attorney to be clear regarding what they are signing. This particular document will become effective when the Principal becomes incapacitated or disabled (permanent or temporary)
The document will require notarization. This completed document may also be revoked at any time the Principal seems necessary.
How to Write
Step 1 – The Parties – Provide the following information as follows:
- To begin, enter the full name of the Principal
- Enter the Principal’s complete address
- AND
- Enter the name of the elected Agent
- AND
- Enter the name of a Successor Agent, in the event the initially elected Agent become unable or unwilling to provide service in this capacity for whatever the reason
- The Principal must carefully review the remainder of the document
Step 2 – Witness by Notary Public –
- The Principal must enter the date of their signature in dd/mm/yyyy format
- In the blank line provided, (under the date) the Principal must, before a Notary, provide their signature
- When the Notary Public has been able to collect identification and witnessed the Principal’s signature, the Notary will then complete all of the required information and provide the official seal
