Georgia Revocation Power of Attorney Form

The Georgia revocation power of attorney form is a document that would be used by a Principal to revoke a power of attorney that had been constructed at an earlier date and is now ready for revocation. The Principal must complete the document, completing all fields and have the document delivered, one copy to the agent and any alternates as well as any other interested parties.

How to Write

Step 1 – Powers to be Revoked – Check the appropriate box(s)

  • Health Care Powers
  • Financial Powers
  • Other – Enter any other titled documents

Step 2 – Parties –

  • Enter the name of the Principal
  • Provide the Document Titles
  • Enter the date in which the document(s) was/were executed
  • Agent’s Name
  • Name of Alternate Agent
  • Enter the date in which the revocation should be recognized

Step 3 – Signature – Before a notary, provide:

  • Print the Principal’s Name
  • The Principal’s Signature

After the completion of the document before a notary, the notary will complete their required information, affixing their official seal (if any)
