Idaho Minor Child Power of Attorney Form

The Idaho minor child power of attorney form is a legal document that provides an avenue in which a parent may place their child(ren) with another person or person’s who are willing to properly care for and make decisions on behalf of, until the parent or parents may safely return them home. This document must be notarized. The document may be revoked at the parent’s discretion in writing and delivered to the temporary guardian.

How to Write

Step 1 – Parent(s) and Children –

  • Type or Print the name(s) of the Parent(s) or Guardian
  • Enter the name(s) of the child(ren)
  • Respective dates of birth
  • Enter the name of the temporary guardian
  • Provide the current address
  • By checking the applicable box, indicate your relationship to the child(ren)
  • The Parent(s) must read and agree to the paragraph regarding powers over the child(ren)
  • Check the box indicating the length of time that the child(ren) shall be in the care of the delegate (enter additional information if needed)

Step 2 – Signature –

  • Parent or Guardian must provide their signature before a notary public

When the notary has witnessed the signature of the parent(s) or guardian, the notary shall complete the remainder of the document in acknowledgement
