Massachusetts Vehicle Power of Attorney Form

The Massachusetts vehicle power of attorney form is a document that would permit a Principal to delegate an Agent to oversee the sale, purchase, inspection,transactions, paperwork and more, related to a motor vehicle(s). This particular document will remain in effect indefinitely or until the Principal revokes the document.

This document must be signed before a Notary Public

How to Write

Step 1 – The Parties –

  • Enter the name of the company or individual who is preparing the document
  • Enter the name of the Agent
  • The Agent must read the information in the first paragraph
  • A company officer or individual must sign at the end of the first paragraph if the Agent is in agreement

Step 2 – If the powers are in the name of an individual, the individual must enter:

  • Their date of birth
  • Social Security Number

Step 3 – If the powers have been placed in the name of a company –

  • Enter the company’s Federal ID Number

Step 4 – Notarization –

Once the Notary witnesses all signatures, the notary will provide all of the information required in order to acknowledge the document
