South Carolina Minor Child Power of Attorney Form | |
The South Carolina minor child power of attorney form is a legal document that parent(s) or a legal custodian may provide to a temporary guardian, the ability to provide signature for emergency health care and/or hospitalization in the absence of the parent(s) or legal custodian, while the child is involved in bowling activities and/or tournaments.
This document may be revoked at any time by the parent(s) or legal custodian, by provision of written notice to the temporary legal guardian.
How to Write
Step 1 – Selection of Temporary Guardian – Enter the name(s) and other required information of the parent(s) or legal custodian(s) as follows:
- Enter the name(s) of the parent(s) or legal custodian(s)
- Complete Address
- Telephone Numbers
- AND
- Authorize a legal Guardian by entering their name into the paragraph
- AND
- Enter the name(s) of the child(ren)
- Enter the date of the document in dd/mm/yy
- Provide the Father’s signature
- Provide the Mother’s signature
- Provide the Legal Guardian’s signature
- Enter the signature of a witness
Step 2 – This section relieves the SC USBC Association of any responsibility if the child(ren) are injured while participating in any activities with this Association:
- Date the section in mm/dd/yyyy format
- Enter the signature of the parent or legal custodian
- Enter the party’s address and telephone number
Step 3 – Temporary Guardianship for Medical Care –
- Enter the name of the temporary guardian
- Provide the name(s) of the child(ren)
- Date the final section in dd/mm/yy
- Enter the Father’s signature
- Enter the Mother’s signature
- Enter the Temporary Guardian’s signature
- A witness must enter their signature in this section
