West Virginia General Financial Power of Attorney Form | |
The West Virginia general financial power of attorney form is a legal document prepared by a Principal to authorize powers to an Agent over their estate. An Agent may then act on behalf of the Principal to perform specified powers stated by the Principal. Powers will begin upon execution of the document and shall remain in effect even if the Principal is no longer able to communicate or direct their own business affairs. It’s strongly advised that a Principal should consult with legal counsel to be certain that the document is fully understood prior to completion. No health care decisions will be granted. This document must be witnessed by a licensed notary public. Any powers document may be revoked, in writing at the discretion of the Principal.
How to Write
Step 1 – Powers – Review the following:

Step 2 – The Parties – Provide:
- Principal’s name
- Street address
- City
- State
- AND
- Agent’s name
- Street Address
- City
- State
- Enter the state whereas financial decisions
- AND
- Successor Agent’s name
- Street Address
- City
- State
- Provide the state of use
- Terms and Conditions
Step 3 – Powers –
- Initial only the powers to be granted on the form
Step 4 – Additional Titles – Review the following:
- Third Parties
- Compensation
- Disclosure
Step 5 – Signatures – Notary Public witness required:
- Principal’s printed name
- Date the signature in dd/mm format
- Principal’s signature
- AND
- Agent’s printed name
- Agent’s signature
- Date signature in mm/dd/yyyy format
- AND
- Successor Agent’s name (optional)
- Successor Agent’s signature
- Date signature in mm/dd/yyyy format
Step 6 – Notary Acknowledgement –
- Once the notary witnesses all signatures, the notary shall enter all required acknowledgement information
Step 7 – Acknowledgement and Acceptance of Appointment as Agent and Successor Agent –
- Each Agent must read their acceptance information:
- Enter the name of the Agents in their respective paragraphs
- Provide signatures
- Date signatures in mm/dd/yyyy formats
Step 8 – Witness Attestation – The witnesses must first review the attestation statement:
- Witnesses must enter their printed names
- Each witness shall provide their signatures
