Wyoming Revocation Power of Attorney Form | |
The Wyoming revocation power of attorney form is a document that is utilized by a Principal who has authorized powers to an Agent, but would now prefer to revoke those powers. These powers may have been authorized for estate management, temporary child care, tax powers or any other powers document. Once the document has been completed, the Principal must have the document delivered or served to the Agent as, until the Agent is in possession of the document, it is not considered effective.
Copies should be sent to all who hold a copy of the powers document ie: banks, Doctors, creditors or any interested party, to make them aware that the Agent no longer has power according to the document they hold.
How to Write
Step 1 – Use of the Document –
- Check the box that would indicate the type of powers to be revoked
- If “Other” is selected, enter the title of the document in the lines provided
Step 2 – The Parties – Enter:
- Principal’s name
- Title of the document to be revoked
- Date of the executed document in dd/m/yy format
- Appointed Agent’s name
- Alternate Agent’s name (if any)
Step 3 – Revocation Date and Signature – To be signed before a notary:
- Enter the date of revocation
- Principal’s signature
- Printed name
Step 4 – Notarization –
- Principal’s signature will be witnessed by a licensed notary
- The notary shall complete the document, validating the document for use
