Michigan Minor Child Power of Attorney Form | |
The Michigan minor child power of attorney form is a document that will permit a parent to provide permission of care to a selected Guardian on behalf of their child(ren) in the absence of the parents. The document will make provisions for all of the children’s basic needs and entertainment. The document will not allow a guardian to sign for a child to marry or sign for adoption. The form will remain in effect for up to six months. The guardian should care for the Parent’s child as if the parent were available to care for and make decisions on behalf of their child(ren) on their own.
This document may be revoked at any time the parent feels they are ready to reclaim their child(ren)
How to Write
Step 1 – Location –
- Enter the child(ren’s) County of residence
Step 2 – Parent/Child(ren) Information – Enter the following:
- The name of the parent(s) who will provide powers to a guardian
- The relationship of the parent to the guardian
- Enter the name(s) of the child(ren) – If more room is required for more information, add a sheet and attach it to this form
- Enter the child(ren’s) date(s) of birth
Step 3 – Guardian’s Information –
- Provide the name of the person delegated to provide temporary care to the child(ren)
- Enter the name(s) of the minor child(ren)
Step 4 – Commencement Date –
- The parent(s) must enter the date in which the children will begin residing with their guardian –
- Read the remaining paragraph
Step 5 – Signatures – Must occur in the presence of a Notary Public –
- Submit the parent’s signature(s)
Witnesses –
- Both witnesses must review the document and then submit their respective signatures
Notary Acknowledgement –
Once the Notary Public has recorded identifications of all signatories and they have signed the document, the Notary shall complete the remainder of the document in acknowledgement
