The Wisconsin general financial power of attorney form is a legal document to be completed by the Principal, so to authorize powers to an Agent over the Principal’s estate. An Agent, would then have the ability to act on behalf of the Principal, to perform specified powers stated by the Principal. Powers may begin upon execution of the signed document. The form will remain in effect even if the Principal is no longer able to direct their own financial business. It would be advised that the Principal consult with an attorney before completing the document. The Agent will be unable to make health care decisions on behalf of the Principal. This document must be witnessed by a notary public. These powers may be revoked, by written notice and delivery from the Principal

Step 1 – Powers – Read:

Step 2 – The Parties – Provide the following information:
- The Principal’s name
- Street address
- City
- State
- AND
- An Agent’s name
- Street Address
- City
- State
- Submit the state from where the financial business will be conducted
- AND
- Provide the name of a successor agent
- Street Address
- City
- State
- Provide the state of use (if needed
- Terms and Conditions
Step 3 – Powers – Review:
- Initial only the powers to be granted on the form
Step 4 – Additional Titles – Read:
- Third Parties
- Compensation
- Disclosure
Step 5 – Signatures – Notary Public witness required:
- The Principal’s printed name
- Date signature in dd/mm format
- Principal’s signature
- AND
- The Agent’s printed name
- Agent’s signature
- Date signature in mm/dd/yyyy format
- AND
- Name of successor agent – (optional)
- Successor Agent’s signature
- Date the signature in mm/dd/yyyy format
Step 6 – Notary Acknowledgement –
- Once the notary witnesses all signatures, the notary will enter all of the state required acknowledgement information
Step 7 – Acknowledgement and Acceptance of Appointment as Agent and Successor Agent –
- The Agent(s) must read their acceptance information:
- Enter the name of the Agents in their respective paragraphs
- Provide Agent signatures
- Date all signatures in mm/dd/yyyy formats
Step 8 – Witness Attestation – The witnesses must first consider the attestation statement:
- Witnesses must enter printed names
- Witnesses must enter their signatures
