Wisconsin Limited (Special) Power of Attorney Form | |
The Wisconsin limited (special) power of attorney form is a legal document which relays in no unclear terms the specialized power bestowed on an Attorney-in-Fact by the Principal. This form is used to ensure that the scope of the authority the Principal wishes to provide the Attorney-in-Fact is well defined and limited to the acts relayed therein. Often a form of this nature will terminate once the assigned duties are completed, or a effective termination date will be added to the document. As relayed in the attached form, the signature of the Principal must be acknowledged by the Principal in the presence of a Notary Public.

Step 1 – Powers – Read:

Step 2 – The Parties – Provide:
- A Principal’s name
- Street address
- City
- State
- AND
- An Agent’s name
- Street Address
- City
- State
- Submit a state where the Agent shall complete the specified acts
- AND
- Name a Successor Agent
- Street Address
- City
- State
- Provide the state of use by this Agent (if needed)

Step 3 – Powers –
- Initial only the powers to be granted leave unauthorized powers boxes, blank
Step 4 – Additional Titles – Review:
- Third Parties
- Compensation
- Disclosure
Step 5 – Signatures – Notary witness required:
- A Principal’s printed name
- Date signature in dd/mm format
- Principal’s signature
- AND
- Agent’s printed name
- Agent’s signature
- Date signature in mm/dd/yyyy format
- AND
- Successor Agent’s name (optional)
- Successor Agent’s signature
- Date signature in mm/dd/yyyy format
Step 6 – Notary Acknowledgement – must be witnessed
- Once the notary witnesses all signatures, the notary shall enter all required acknowledgement information
Step 7 – Acknowledgement and Acceptance of Appointment as Agent and Successor Agent –
- Agent must read the acceptance:
- the name of the Agents in their respective paragraphs
- Agent’s signatures
- Date of signatures in mm/dd/yyyy formats
Step 8 – Witness Attestation – The witnesses must review the attestation:
- Witnesses must enter their printed names
- Each witness shall provide their signatures
