Wisconsin Real Estate ONLY Power of Attorney Form

The Wisconsin real estate only power of attorney form is a document that shall be completed by a Principal owner of real property to provide authorization to an Agent so that they may perform specific acts pertaining to the real property of the Principal.

The document will become effective on the date of execution and will remain in effect until either the specified acts are completed or the Principal revokes the document. The form may remain in effect if the Principal becomes disabled or incapacitated. This document must be notarized.

How to Write

Step 1 – The Parties – Enter the following information:

  • Full name of the Principal
  • Street address
  • City
  • State
  • AND
  • Agent’s name
  • Street address
  • State

Step 2 – Assignment of Authority –

  • Check any or all of the boxes representing the powers the Principal would like to grant to the Agent:

Step 3 – Term –

  • Select and check only one of the boxes that best indicates the Principal’s preferred term

Step 4 – Signatures/Revocation – Provide the following information:

  • Date execution of the document in dd/mm/ yy format
  • Signature of Principal
  • Printed name
  • AND
  • Agent’s signature
  • Printed name

Step 5 – Witness Affirmations – Witnesses must read and agree to the affirmation:

  • Witnesses names
  • Witnesses signatures
  • Printed names

Step 6 – Notarization –

  • The notary shall witness all signatures
  • Notary will complete their required information, acknowledging the document
