The Idaho revocation power of attorney form is a form that is used by a Principal who has prepared a power of attorney but is ready to revoke the document. The form is straight forward and once completed and notarized, may be delivered to any/all Agent’s named on the original document. The revocation may become immediately by entering the chosen date into the document.
How to Write
Powers to be Revoked – Check the applicable box(es)
- Health Care Powers
- Financial Powers
- Other – Enter any other titled documents
Step 2 – The Parties – Provide the following information:
- Provide the name of the Principal
- Enter Document Titles
- Provide the date in which the document(s) were executed
- Enter the Agent’s Name
- Enter Name of Alternate Agent
- Enter the date in which the Principal would like to make the document effective
Step 3 – Signature – In the Presence of a Notary:
- Print Principal’s Name
- The Principal’s Signature
After the signing of the document before a notary, the notary will complete their required information, in acknowledgement, the notary will affix the original seal
