Maine Revocation Power of Attorney Form | |
The Maine revocation power of attorney form is a document that will permit a Principal or Parent of a minor child in the care of a guardian, to revoke the powers granted and either initiate a new powers document or regain control of all finances and property or their children. Simply complete the document with the required information on the form and deliver it to the Agent or Guardian.
This document will require notarization. The form may be revoked at the will of the Principal and/or parent(s).
How to Write
Step 1 – Download the document – Check the boxes that apply with regard to a health care powers document:
- Health Care Powers
- Financial Powers
- Other – enter any other powers that would require revocation in the line provided (ie: any POA or Minors documents)
Step 2 – Principal’s Information –
- Enter the name of the Principal
- Enter the title of the document for revocation
- Enter the date of execution of the document
- AND
- Enter the name of the Agent
- The name of the Alternate or Successor Agent (if any)
- Provide the date of revocation
Step 3 – Signatures – this document must be signed before a notary public:
- The Principal must enter their Signature
- Enter the Principal’s printed name
Step 4 – Notary Acknowledgement –
As the Principal or Parent completes the document and provides signature(s), the notary will complete the remainder of the document to make it available for use. The Principal may wish to make copies and make them available to anyone holding a copy of the document and keep the original for record keeping
