Utah Vehicle Power of Attorney Form | |
The Utah vehicle power of attorney form is a document that is prepared by a Principal who would like to choose an Agent to act in all business pertaining to motor vehicles and/or power equipment. Duties would include, titling, registration, preparation of any documentation, ownership transference of any motor vehicle or power equipment that the Principal owns, rents, sells or that requires any other action required in the absence of the Principal.
How to Write
Step 1 – Appointment of Agent – The Principal must read the paragraph and enter:
- The Company and/or Principal’s name
- The name of the Agent
Step 2 – Signature – Submit the following:
- The signature of the Principal or the Authorized Company signatory
Step 3 – Individual Principal (if the principal is not a company) – Provide:
- Submit the Principal’s date of birth in mm/dd/yyyy format
- Social Security Number
- OR
- If the Principal is a company, enter the Federal Identification Number (FIN)
Step 4 – Notarization –
- Upon witness of signatures, the notary will complete the document in acknowledgment and by affixing the Notary Seal
