The Florida vehicle power of attorney form is a legal document that is used by the individual(s) granting powers, to authorize the attorney-in-fact/agent. to act on his/her behalf in matters related with a motor vehicle, mobile home or any other type of motorized vessel. By providing this document to your attorney in fact/ agent, they will be able to legally act on your behalf with regard to any responsibilities needed to properly acquire any motorized vehicle by performing title transfers either way, signature on your behalf or any other state requirements. This document may be revoked at any time in writing and delivery to your selected attorney in fact/agent. (See 49 C.F.R. ss. 580.4 and 580.13)
How To Write
Step 1 – Begin by downloading the document:
- In the first line at the top left of the page, enter the date (mm/dd/yyyy format) in which the attorney in fact/agent is being assigned and the document is being completed
- On the second line at the top of the page, the Principal will appoint an attorney in fact/agent, by placing that person’s name on the line in the first paragraph. Read the paragraph first and then enter the name of the principal’s selection of whom you’ve chosen as your attorney in fact/agent
Step 2 – Vehicle/Vessel Information – In this section the principal shall enter the appropriate information in each box provided as follows:
- First check one of the boxes on the form stating what type of vehicle for which you are providing power to your agent –
- Motor Vehicle
- Mobile Home – or
- Vessel
- Next, provide –
- The Year
- Make/Manufacturer
- Body Type
- Title Number
- Vehicle/Vessel Identification Number also known as the VIN
Step 2 – It will be necessary for the owner and the co-owner (if applicable), to carefully read the entire document to be certain everything is correct, before you provide any signatures – Once you’ve read all of the information and believe it to be true in content, provide the following information:
- Signature of Owner “Grantor”
- Legibly Printed Name of Owner”Grantor”
- Driver License, Identification Card or FEID Number for Owner
- Date of Birth for Owner
- Owner’s physical address
- City
- State
- Zip Code
- AND (if applicable)
- Signature of Co-Owner “Grantor,” if applicable
- Legibly Printed Name of Co-Owner “Grantor,” if applicable
- Driver License, Identification Card or FEID Number for Co-Owner
- Date of Birth for Co-Owner, if applicable
- Co-Owner’s physical address
- City
- State
- Zip Code
Step 3 – Review the information in the final paragraph on the form. If you are in agreement your document is complete. The online information has a link at the bottom that will take you to addresses for all locations in the state of Florida. Select your city and select an address. You may take it in for approval or you may send it by USPS once your transaction is complete. Make copies for anyone who has signed the document. This original document must be sent or taken to the DMV and submitted with the title.
