Louisiana General Financial Power of Attorney Form | |
The Louisiana general financial power of attorney form is a legal document that permits a Principal to specify general powers over the finances of the Principal by an Agent that has been specifically selected by the Principal. The Agent should be aware that the Principal, although the Agent will be granted unlimited financial powers, will always continue to retain complete rights over their finances and property at all times. As well, as the document is authorized for the Agent to become a part of, the powers will not remain in effect should the Principal become incapacitated.
The Agent will not have the authorization to make medical decisions on behalf of the Principal The Principal may impose restrictions and limitations with regard to this authorization. The Principal may revoke this document at any time they wish by either, creating a new document or by placing notice into writing and having the document delivered or served to the Agent.
How to Write
Step 1 – Titled Sections – The Principal must begin by reviewing the following titles:

- Incapacitation
- Revocation
- Witness and Notary
Step 2 – The Parties – Enter:
- The name of the Principal
- Principal’s current physical address
- City
- State
- AND
- The Agent’s full name
- Physical Address
- City
- State
- AND – The Principal may wish to appoint a Successor Agent in the event the initial Agent is unwilling or unable to continue to serve in the role of Agent, on behalf of the Principal
- Enter the name of the Successor Agent
- Physical Address
- City
- State
- The state where the document will be executed

Step 4 – Signatures – Signatures of all parties must be entered before a Notary Public – Entered the following:
- The printed name of the Principal
- Date of the Signature in dd/mm
- Enter the Principal’s Signature
- AND
- The delegated Agent’s printed name
- Read the remainder of the information provided
- Agent must submit their signature
- Date the signature in mm/dd/yyyy format
- AND
- The printed name of the Successor Agent
- Read the remainder of the information’
- Submit the Successor’s Signature
Step 5 – Notarization –
Once the Notary has witnessed all of the signatories and signatures, they will then provide all of the state required licensed, notary public information and shall affix the notary’s state seal (if any)
Step 6 – Acknowledgement and Acceptance of Appointment as Agent(s) Both the initially selected Agent as well as the possible Successor, must provide the following:
- Printed name of each Agent
- Read the remaining portions of each section, respectively
- Both Agent’s must submit their signatures
- Both must date their signatures in mm/dd/yyyy format
