California Vehicle Power of Attorney Form

The California vehicle power of attorney form is a document that allows an Attorney in Fact/Agent  to complete all necessary documents, as needed, to transfer ownership of a vehicle as required by California laws and regulations. This document has two sections that are identical and both of them need to be filled out in ink. The bottom form must be cut off and retained by the principal. (See REG 260)

How To Write

Step 1 – Begin by downloading the form and entering the following information under Vehicle/Vessel Description:

  • Vehicle Identification Number (VIN)
  • Year Model
  • Make
  • License Plate/CF Number
  • Motorcycle Engine Number

Step 2 – Print the name or names of the principal(s)

  • Print the name of the appointee

Step 3 – Signatures – ALL signatures must be signed in ink on both copies of this document –

  • The next two lines are for signatures required (in ink) that are required by the person or persons appointing the power of attorney. Also provide for each signature:
  • City
  • State
  • Zip Code
  • Driver’s license, ID or Dealer Number

The final copy must be completed identically to the top copy and must be cut off and retained by the person(s) appointing the document. Provide the first copy to your Attorney in Fact/Agent
