Utah Revocation Power of Attorney Form | |
The Utah revocation power of attorney form is a document that will allow Parent(s) or Guardian to revoke a previously written powers document and therefore resume the rights and responsibilities of their children. This document shall keep the parent(s) or guardian in compliance with a Minor Child powers document in provision of written notice of revocation. The form must be delivered or served to the Temporary Guardian before the revocation will be considered in effect.
The document, once complete, will require the witness of a Notary Public.
How to Write
Step 1 – Download the Document – Enter the following required information:
- The date that the powers document was executed in mm/dd/yyyy format
- Enter the name of the appointed temporary guardian
- Provide the name(s) of the child(ren)
Step 2 – Signature – Must be entered before a Notary Public – Submit the following:
- The parent(s) or guardian must read the revocation statement
- Date the signature in mm/dd/yyyy format
- Parent or Guardian must provide their signature
- Print the name of the person providing signature
Notarization –
- After the signature has been witnessed by the Notary Public, the Notary will complete the document in acknowledgement also affixing their official Notary Seal to the document
