The Washington revocation power of attorney form is a legal document that allows a Principal, who has provided a powers document to an Agent or Agents to act on their behalf, in the event of their absence or illness. Once the Principal is able to again make their own decisions, or if the Principal finds they no longer need the services of the Agent(s), they may complete this form, sign before witness and either deliver or have it served to the Agent(s). Be advised that the document is not effective until it’s in the possession of the current Agent.
How to Write
Step 1 – Filed for Record – Submit the following:
- Enter the name and address of the party who has requested filing of the revocation
Step 2 – Revocation – Enter:
- Agent(s) names who shall receive this revocation
- Date of revocation in mm/dd/yyyy format
- Principal’s Signature
Step 3 – Notarization –
- The notary shall witness the signature and complete the remainder of the document with the required information
- The notary shall also affix their official state seal
