The Wyoming minor child power of attorney form is a document that is provided by parent(s) or guardian to provide temporary powers over the child(ren) in their care, to a temporary custodian in the event of absence or illness. The document must be witnessed and may be revoked in writing by the parent(s) or guardian as they wish.

Step 1 – Child(ren)/Parent(s)/Guardian(s) Information –

Step 2 – Appointment of Temporary Custodian –
- Enter the name of the Attorney In Fact/Temporary Custodian
- Relationship to the child(ren)

Step 3 – Delegation of Powers –
- Initial and check the box that indicates the type of powers granted
Step 4 – Commencement of the Document –
- Enter the date in dd/m/yy format
- Initial and check one box indicating the commencement
Step 5 – Signatures – To be signed in the presence of a notary
- Parent’s/Court Appointed Guardian’s Signatures
- Printed names
- Date of signatures in mm/dd/yyyy format
Step 6 – Affirmation by Witnesses –
- Witnesses must enter their names in their respective paragraphs
- Read and agree to the affirmations

Step 7 – Notary Acknowledgement –
- Signatures shall be witnessed by a notary
- Once witnessed, the notary shall provide all remaining information in acknowledgement
