Iowa Revocation Power of Attorney Form | |
The Iowa revocation power of attorney form is a legal document that will record the revocation of a POA form that had previously been implemented. Completion of this form shall revoke the powers provided to the Agent(s), immediately upon the stated date entered into this document. The Principal may complete and sign the form and deliver it to the Agent(s) and any other interested parties.
This document will require notarization to become effective.
How to Write
Step 1 – Download the document and select the boxes that apply, as follows:
- Health Care Powers
- Financial Powers
- Other (enter the titles of any other powers documents)
Step 2 – Principal and Agent – Enter the following:
- The name of the Principal
- Provide the title of the document
- Enter the date of execution of the document to be revoked
- AND
- Enter the name of the Agent
- Alternate Agent’s name (if any)
- Enter the desired date for revocation
Step 3 – Signature – Must be signed before a notary public
- Enter the printed name of the Principal
- Principal must enter their Signature
Step 4 – Notary Acknowledgement
- The Notary Public shall witness the signature of the Principal, complete the notary information, in acknowledgement
Copies of the revocation should be provided for anyone who holds a copy of the powers document. The Principal should retain the original copy for their own records.
