Iowa General Financial Power of Attorney Form

The Iowa general financial power of attorney form is a document that complies with Iowa state law with regard to the appointment, by a Principal, of an Agent. This Agent will be granted permission of powers to make decisions regarding the financial matters of the Principal, This document will not remain in effect in the event of incapacitation. With this document, the Principal remains in control of their finances. The Principal may impose restrictions allowing the Agent only specific powers. Once this document has been signed and becomes effective, any other powers document will be immediately invalid.

This form will require notarization. As well, the Principal may revoke this document at their discretion, by placing a revocation notice into writing serving it to the Agent(s).

 

How to Write

Step 1 – Titled Sections – The Principal must begin by reviewing the following titled sections:

  • Notice
  • Incapacitation
  • Revocation
  • Witness and Notary

Step 2 – Principal’s Information – Enter:

  • The name of the Principal
  • Street Address
  • City
  • State
  • AND
  • The name of the Agent
  • Street Address
  • City
  • State
  • Enter the state where POA will be executed
  • AND
  • (Optional) Select a Successor Agent and enter their name
  • Street Address
  • City
  • State
  • Enter the state where POA will be executed

Step 3 – Titled Sections – Principal must review the additional group of titled sections as follows:

  • Terms and Conditions
  • third Parties
  • Compensation
  • Disclosure

Step 4 – Signatures – Before a Notary Public, enter the following information:

  • Enter the Printed Name of the Principal
  • Enter the Date of the Signature in dd/mm/yyyy format
  • Enter the Principal’s Signature
  • AND
  • Enter the Printed Name of the Agent/Attorney In Fact
  • Agent must read the entire paragraph
  • Agent must enter their signature
  • Date the signature in mm/dd/yyyy format
  • AND
  • Enter the Name of the Successor Agent
  • Successor must read the entire paragraph
  • Enter the Successor Agent’s  Signature
  • Date the Signature in mm/dd/yyyy format
  • AND
  • Once the Notary has witnessed all signatures, the Notary will complete their required information and affix the official seal

Step 5 – Acknowledgement and Acceptance of Appointment by the Agents – Both Agents must review their respective paragraphs and enter the following:

  • Agent’s Name
  • Signature of Agent
  • Date of Signature in mm/dd/yyyy format
  • AND
  • Successor Agent’s Printed Name
  • Signature of the Successor Agent
  • Date the Signature in mm/dd/yyyy format
