The Indiana general financial power of attorney form is a document that grants the Principal the lawful right to transfer unlimited financial powers to the Agent the Principal will select. The document will not remain in effect if the Principal becomes disabled or incapacitated. The Principal may choose to limit or restrict some powers. This document should be read carefully by the Principal to be certain that they understand all aspects of the document. If the information is confusing, the Principal may wish to consider a consultation with an attorney for clarification.
This document will require witness signatures or notarization. The Principal may revoke this document at their own discretion,
How to Write
Step 1 – Introduction – The Principal should read the following before proceeding:

- Incapacitation
- Revocation
- Witness and Notary
Step 2 – Principal’s Information –
- Enter the name of the Principal
- Street Address
- City
- State
- AND
- Enter the name of the Agent/Attorney In Fact
- Street Address
- City
- State
- AND
- Enter the state where the document is being drawn
- AND
- Enter the name of a Successor Agent
- Street Address
- City
- State
- The Principal should then review all of the following Titled Sections:
- Terms and Conditions
- Third Parties
- Compensation
- Disclosure
Step 3 – Signatures – All signatures must be entered before a Notary Public
- Provide the Printed Name of the Principal
- Date the signature in dd/mm format
- Enter the signature of the Principal
- AND
- Printed name of the Agent
- The Agent must read the paragraph pertaining to appointment
- Enter the Signature of the Agent
- Date the Signature in mm/dd/yyyy format
- AND
- Printed name of the Successor Agent
- The Successor must read the information pertaining to the Successor Agent’s duties
- The Successor Agent must enter their Signature
- Date the Signature in mm/dd/yyyy format
When the notary or witnesses have had the opportunity to attest to all signatures, they will provide all of the information required to acknowledge the document
- Agent and Successor Agent’s must review the acceptance information
- Enter the printed names of the Agent and Successor (respective)
- Each must enter their signatures
- Date the signatures in mm/dd/yyyy format
- Witnesses must read the Attestation, if the witnesses agree, enter the signatures of the Witnesses
