The Illinois vehicle power of attorney form is a document that is provided to an Agent by a Principal (owner) to grant powers that would allow the Agent to act on behalf of the Principal during the transactions involving motor a motor vehicle.
The Principal may allow powers so that an Agent may sign all papers and documents required to secure Illinois title and/or registration and/or transfer interest in the Principal’s vehicle(s).
How to Write
Step 1 – Principal’s Information –
- Name of Principal
- Address
- AND
- Agent’s Name
- Address
Step 2 – Vehicle Information – Enter the following:
- Make
- Year
- Model
- Body Type
- Vehicle Identification Number (VIN)
Step 3 – Purchaser’s Information –
- Purchaser’s Name
- Address
- Date of Sale in mm/dd/yyyy format
Step 4 – Signature –
- Principal’s Signature
- Date of Signature in mm/dd/yyyy format
