Indiana Vehicle Power of Attorney Form

The Indiana vehicle power of attorney form is a document that provides permission to any delegated Agent, to transact any deal with regard to the Principal’s vehicle and/or watercraft. The document will be valid to the Agent for 90 days from the date of the instrument. this document may be revoked as the Principal wishes.

How to Write

Step 1 – Principal, Agent, Vehicle and/or Watercraft Information  – Enter the following:

  • The name of the Principal or Agent
  • Principal’s Address
  • County
  • State
  • AND
  • The name of the delegated Agent
  • Vehicle Identification Number (VIN) or Hull Identification Number (HIN)
  • Make of the Vehicle or Watercraft
  • Year Title Number
  • Customer’s Signature
  • Customer’s Printed Name
  • Date of Signature in mm/dd/yyyy format

Step 2 – Notary Certificate –

  • Once the document is completed, the notary will complete their portion of the form to acknowledge the document has been signed by the stated signatory

Step 3 – Principal, Agent, Vehicle and/or Watercraft Information  – Enter the following:

  • The name of the Principal or Agent
  • Principal’s Address
  • County
  • State
  • AND
  • The name of the delegated Agent
  • Vehicle Identification Number (VIN) or Hull Identification Number (HIN)

  • Year Title Number
  • Customer’s Signature
  • Customer’s Printed Name

Step 4 – Notary Certificate –

  • Once the document is completed, the notary will complete the notary public portion of the form to acknowledge the document has been signed by the stated signatory
