Minnesota Vehicle Power of Attorney Form

The Minnesota vehicle power of attorney form is a document that would be completed by the Principal (owner/title holder) of a vehicle or vehicles to appoint powers to a selected Agent. The document is designed as such, that it will be effective indefinitely or until revoked, in writing by the Principal. Provision of this document to the Agent would allow the Agent to purchase, sell, have inspected or do transactions in the absence of the Principal just as if the Principal were present to conduct the business on their own.

This document must be signed by the parties, before a notary. This form may be revoked at the discretion of the Principal, by placing notice in writing and delivery to the Agent.

How to Write

Step 1 – Parties –

  • Enter the name of the Principal
  • AND
  • Enter the name of the Agent to whom powers shall be granted
  • Carefully review the remainder of the paragraph

Step 2 – Signature – Must be signed before a Notary Public

  • The document must be signed by an individual or an authorized officer of the company
  • Provide the appropriate signature

Step 3 – Assignment of Powers -Should the Agent be an individual, the Agent must provide the following:

  • Agent’s date of birth in mm/dd/yyyy format
  • Agent’s Social Security Number
  • OR
  • If the Agent is in the name of a Company provide their Federal Identification Number

Step 4 – Notarization –

As the Notary Public has witnessed the signature of the document, they shall complete the remainder of the form with the required information that will acknowledge the document

Step 5 – Inspection Sheet – If the state requires that a seller provide a safety inspection, print the second page of the document and take it to a licensed inspection station

  • Once all of the items on the list have been properly inspected and passed inspection, the completed form may be given to the buyer upon purchase.
