Motor vehicle power of attorney is a way for an owner of a vehicle to grant specific or broad responsibility to an individual over the registration, titling, and the purchase and/or sale. The agent, or ‘attorney-in-fact’, that is selected by the owner of the vehicle is allowed to sign on their behalf as long as an original document or certified copy is in their possession. In addition when signing any documents, the words ‘acting as POA’ or any synonymous variation of these words should be used.

How to Write
Step 1 – The Parties – Enter the following:
- Name of individual or company
- Complete mailing address
- AND
- Principal’s name
- Complete mailing address
- The state where the document shall be executed
Step 2 – Vehicle Information – As follows:
- Year
- Make
- Model
- Style
- Vehicle Identification Number (VIN)
Step 3 – Additional Information – In the event the document is in the name of an individual, enter the following:
- Individual’s date of birth in mm/dd/yyyy format
- Social Security Number
- AND
- If the document is in the name of a company, provide the Federal Identification Number or the Employer Identification Number
Step 4 – Signature –
- Submit the Principal’s signature
- Date the signature in mm/dd/yyyy format
